Services will include:

• A one-on-one interaction with each community member visiting MOBEC to assess risk factors for diabetes.
• Diabetes screening and individualized education to prevent and manage diabetes and prediabetes.
• Blood pressure screening/checks and education
• Cholesterol screenings
• Resources for tobacco cessation, food distribution, transportation, and other community services as needed.
