Title: Optimizing Practice to Coincide with Patients' Social Determinants of Health
Presenter: Shane Desselle, RPh, PhD, FAPhA
Objectives: Identify individual and interpersonal theories of health and illness behavior.
Analyze the relationship between diagnosis and patient care strategies to optimize therapeutic outcomes.
Apply empathy to assess patients' health needs, considering the whole person and the influence of social, environmental, and psychological determinants beyond personal actions.
Describe how various determinants of health can alter your approach to care delivery.
Describe the utility of models, in general, and specific behavioral models explaining patients’ beliefs, behaviors, and consumption of health care resources. Apply various models of care to improve your ability to develop a therapeutic, covenental relationship with patients to acquire their trust.
